PocDoc partners with Hartlepool & Stockton Health GP Federation to deliver essential health checks to local communities in Hartlepool and Stockton-on-Tees as part of a nationally funded programme to increase cardiovascular disease prevention across the UK
PocDoc is excited to announce that we have partnered with Hartlepool & Stockton Health GP Federation to deliver essential health checks to local communities across Northeast England. This partnership represents PocDoc’s commitment to ensuring that health screenings are accessible to all individuals nationwide and follows announcements of PocDoc’s recent involvement in a similar programme funded by the Department of Health and Social Care in Cumbria.
Similar to Cumbria, Hartlepool and Stockton are rural areas in NorthEast England with populations that have been identified as potentially benefitting significantly from more accessible health screenings. Increased access and awareness to heart health screenings, especially in areas of high deprivation, aids in the overall reduction in the prevalence of cardiovascular disease—which is 80% preventable, according to the WHO.
Together, PocDoc and their NHS partners in Hartlepool and Stockton Heath will provide entire communities with complete digital heart health screenings while increasing awareness of combating non-communicable diseases. This partnership represents the initial step in further rolling out a national digitally enabled community-first model of care.

PocDoc’s Healthy Heart Check
The Healthy Heart Check, developed by PocDoc, offers a convenient solution to accessible healthcare. Using PocDoc’s microfluidic assay technology, including the company’s patented HUESnap® AI platform, and a smartphone, PocDoc allows patients to receive an NHS-equivalent cardiovascular health screening in just 10 minutes.
The PocDoc Healthy Heart Check gives patients results on their full lipid panel, QRISK score (which includes their NHS Heart Age and 10-year risk of having a cardiovascular event), blood pressure and BMI. The app will display the results along with comparisons with NHS guidelines.
PocDoc’s partnership with Hartlepool and Stockton Health will also allow patients to discuss their results with a clinician. During this process, they’ll be offered lifestyle advice and resources to help them understand their results.
