PocDoc partners with Thames Valley GP practice on cardiovascular disease

Digital health diagnostics company PocDoc has partnered with Unity Health, a five-site GP practice in the Thames Valley, to identify patients at high risk of developing cardiovascular disease across the region.

PocDoc, the UK's leading digital diagnostics company, has signed a first-of-its-kind agreement with Unity Health that gives the practice unlimited use of the PocDoc Healthy Heart Check across all five of its surgeries. From 1 September 2026, clinicians at Unity Health's sites in Brill, Chinnor, Long Crendon, Princes Risborough and Thame will be able to offer cardiovascular screening to patients whenever clinical need calls for it, without the cost of an individual test shaping who gets tested.

The partnership is designed to help the practice proactively find cardiovascular risk across its patient population, particularly among the groups least likely to be picked up through traditional pathways, and to intervene before serious conditions develop.

Each Healthy Heart Check delivers a full cholesterol profile, body mass index, NHS Heart Age and a ten-year cardiovascular risk assessment from a single finger-prick sample, with results returned in minutes through the patient's own smartphone. Crucially, results are returned to Unity Health through GP Connect, ensuring the GP record remains the single source of truth and that the check complements, rather than complicates, a patient's existing care.

The model itself is what makes this partnership significant. It is the first of its kind for PocDoc, giving a GP practice unlimited CVD testing under a single annual agreement. That structure removes a subtle but powerful barrier to prevention: when every test carries an individual cost, the decision to screen can become a decision about budget. Under this agreement, the decision to test is based purely on patient need.

Both partners have committed to a full joint evaluation of the programme, which will be published and shared with the wider system, adding to the growing evidence base for how digital diagnostics can strengthen cardiovascular prevention in primary care.

The partnership directly supports the Government's 10 Year Health Plan and reflects the NHS's three key shifts in action: from hospital to community, from sickness to prevention, and from analogue to digital. It embeds rapid, point-of-care cardiovascular screening within primary care, identifies risk before symptoms appear, and does so through a fully digital pathway that writes back into the patient record.
