PocDoc partners with Second Nature to give people a next step the moment they see their heart age
PocDoc, the UK's leading digital diagnostics company, has partnered with Second Nature, one of the UK's largest digital weight management providers. People whose Healthy Heart Check results show they could benefit from support with their weight can now access Second Nature's programme, if eligible.
The moment that matters
Picture someone in their late forties opening an app and reading that their heart is ten years older than they are. That number lands. For a lot of people it's the first time their risk has felt real, and it's often the moment they're most ready to do something about it.
Motivation like that fades fast if there's nowhere to take it. This partnership gives it somewhere to go. When a Healthy Heart Check result points to weight as part of the picture, the person can be connected to Second Nature's support straight away, while the result is still fresh.
Second Nature's evidence-based programme pairs an app with support from doctors, dietitians and psychologists, plus medication where clinically appropriate. It has helped more than 300,000 people build habits that last.
Why it matters
Two in every three adults in England are overweight or living with obesity, and three in ten are living with obesity, according to the Health Survey for England 2024. Obesity rates have doubled since 1993.
Heart disease, stroke and type 2 diabetes are among the UK's leading preventable diseases, and weight is one of the risk factors people can change most.
Built for the 10 Year Health Plan
The partnership reflects all three shifts set out in the government's 10 Year Health Plan. It moves care from sickness to prevention, by acting on risk before it becomes a heart attack, stroke or diabetes diagnosis. It moves care from analogue to digital, with testing, results and support all delivered through a smartphone. And it moves care from hospital to community, reaching people at home rather than waiting for them to arrive in a clinic.
What the founders say
Chris Edson, CEO and Co-founder of Second Nature, said:
"Seeing your heart age in black and white is a powerful moment, and it's often when people are most ready to make a change. PocDoc reaches them right then. Together, we can make sure that motivation turns into support that lasts."
Steve Roest, CEO and Co-founder of PocDoc, said:
"Identifying cardiometabolic risk is only valuable if it connects people to the right support. Weight is one of the biggest factors people can change, and Second Nature brings proven expertise in tackling it. PocDoc has built the infrastructure to identify that risk at scale, and to give people a clear next step as soon as they see their results."
