PocDoc and Patients Know Best partner to bring home heart screening service to the NHS App

In a new partnership announced today, patients using the PocDoc Healthy Heart Check can now seamlessly access and share their screening results through Patients Know Best’s (PKB) personal health record platform. With this partnership patients can see all their data through the NHS App, supporting the government’s vision for a single place for people to manage their health.

Globally, Cardio, Metabolic and Renal (CMR) diseases are the leading cause of ill-health and death. In the UK alone, cardiovascular disease affects around 7.6 million individuals. Promisingly, 80% of cardiovascular disease-related cases could be prevented, saving lives and financial resources1.

Early detection and preventive care are key to addressing this challenge and align with the UK’s government’s stated shift to a more preventative healthcare model. PocDoc’s Healthy Heart Check provides patients with a full cholesterol profile, body mass index score, NHS heart age and a ten-year risk assessment for heart attacks and strokes, all delivered in under 10 minutes.

With the PKB integration, coded screening data is automatically sent to a patient’s personal health record. Any patient using PocDoc can securely share their Healthy Heart Check results with their GP or other healthcare professionals, ensuring timely interventions where needed. The partnership not only enhances individual patient care, but also supports population health management through coded, structured data delivery at scale.

By storing screening results alongside a patient’s complete health record, the service empowers individuals with a holistic view of their health. It encourages proactive engagement in managing overall well-being while accelerating the shift from hospital-centric care to more accessible, preventive healthcare services at home or in the workplace.

This service is accessible for patients of the 22 ICS (Integrated Care Systems) regions where PKB is live in the NHS App.
